Georgia Vaccine Exemptions: Simple Guide for Parents

In Georgia, parents can legally opt out of school and daycare shots by filing one form: DPH Form 2208 (Affidavit of Religious Objection to Immunization).

✅ Steps to Claim Your Exemption

  1. Get the form
    • It’s called Affidavit of Religious Objection to Immunization (DPH Form 2208).
    • Schools and childcare centers should give you a copy, or you can print it yourself (we’ll link the PDF here).
  2. Fill it out
    • Write your child’s full name and date of birth.
    • Sign your name as the parent/guardian.
    • Swear or affirm that vaccines are against your religious beliefs.
  3. Notarize it
    • The form must be notarized. This means you sign it in front of a notary public (many banks, UPS stores, or libraries have one).
  4. Submit it
    • Give the completed, notarized form to your child’s school or daycare.
    • Keep a copy for your own records.

That’s it. No yearly renewals. No extra paperwork.
