The strength of the patient - physician relationship is vitally important in maintaining good medical care. Due to the failure to comply with our vaccination policy, that relationship has deteriorated such that it would be in the best interest of all for you to find another health care provider.

If you so desire, I will continue to provide your care for thirty days of this letter. This will give you ample time to select a new physician of your choice from the several other competent practitioners in our area. When you have selected a new physician, please sign and complete a records release form and we will be happy to forward your medical records to your new physician’s office.
